High numbers of red blood cells may indicate:

Cigarette smoking
Low oxygen tension in the blood
Congenital heart disease
Cor pulmonale
Pulmonary fibrosis
Polycythemia vera
Dehydration (such as from severe diarrhea)
Emphysema
